Service interest<\/p>\n<p>* Denotes information typically required to provide a requested service.<\/p>\n<p>1.2. We may also receive the following types of information in connection with provision of Morphic\u2019s Services and Products:<\/p>\n<p>1.2.1. Patient name<\/p>\n<p>1.2.2. Medical record number<\/p>\n<p>1.2.3. Research subject ID<\/p>\n<p>1.2.4. Patient\/subject Initials<\/p>\n<p>1.2.5. Medical imaging<\/p>\n<p>1.2.6. Operative information<\/p>\n<p>1.2.7. Patient demographics (e.g., age, sex, race, height, weight, etc.)<\/p>\n<p>1.2.8. Transaction-related details<\/p>\n<p>1.3.\u00a0\u00a0\u00a0 Collection of Personal Information.\u00a0 We collect Personal Information in a variety of ways, including:<\/p>\n<p>1.3.1. Through the Services.\u00a0 We collect Personal Information through the Services, for example, when you request information regarding the services we provide; or register for an account to access the Services, enter into a transaction with us, or contact us for support.<\/p>\n<p>1.3.2. From Other Sources.\u00a0 We may receive Personal Information from other sources in connection with clinical research studies or other Morphic products or services. For example:<\/p>\n<p>1.3.2.1.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Clinical trials and other research studies<\/p>\n<p>1.3.2.2.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Third-parties such as CROs and clinical sites<\/p>\n<p>1.3.2.3.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Physicians and other healthcare providers<\/p>\n<p>1.3.2.4.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Publicly available databases<\/p>\n<p>We need to collect Personal Information in order to provide certain Services. If you do not provide the information requested, we may not be able to provide the Services. If you disclose any Personal Information relating to other people (e.g., patients, research subjects, your partners or vendors, etc.) in connection with the Services, you represent that you have the authority to do so and to permit us to use the information in accordance with this Privacy Policy. Morphic will only process your Personal Data for the purposes described in this Privacy Policy or as otherwise permitted under applicable law, including the General Data Protection Regulation (EU) 2016\/679 of 27 April 2016 (\u201cGDPR\u201d).<\/p>\n<p>1.4.\u00a0\u00a0\u00a0 Use of Personal Information. For example, when you request information about our products and Services.<\/p>\n<p>1.4.1.3.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 To enter into transactions, verify your information, and provide you with related benefits and support.<\/p>\n<p>1.4.1.4.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 To send administrative information to you, such as changes to our terms, conditions, legal agreements, and policies.<\/p>\n<p>We will engage in these activities to manage our contractual relationships and\/or to comply with a legal obligation.\u00a0 Art. 6 para. 1 lit. We may use and disclose Other Information for any purpose, except where we are required to do otherwise under applicable law. If we are required to treat Other Information as Personal Information under applicable law, we may use and disclose it for the purposes for which we use and disclose Personal Information as detailed in this Policy.
